THIS IS THE MOST HORRIBLE PLACE! Staff as well as the case managers are unprofessional! I don’t recommend sending loved ones to this trash of a place. The night my grandpa passed away we were allowed to go and the lobby smelled like feces. No one in the front to greet you! In the back was only two staff members that I saw! The whole building was gross!!

Staff was excellent, especially Andrea and Annie. I was treated like a queen. Got meds when I was suppose to. Physical therapy was wonderful, it helped me a lot. Going to continue therapy.

Incompetent and non-caring staff about sums up our experience. Do not send your loved ones here if at all possible - I normally don't write these but don't want anyone else to go through the experience we did with our loved one. My father in law was placed here as he had contracted COVID from another rehab facility - we had no choice but to send him here. The list of grievances is far too great to list here. To name a few, his second night there he fell and the staff failed to mention this to us - He had just recovered from major surgery!! Was always a challenge to get hold of anyone and get answers - plus the arranged visits (through a glass) were a joke. They often didn't wear masks/gloves - which is concerning being a facility that only accepts COVID patients. We ended up having to remove him from there (with COVID) as things got very bad. We learned a few days later that the staff had told him never to ring his call button - UNBELIEVABLE!! It was very apparent they took advantage of a fragile old man and the fact that no one could be inside the facility. I have read the previous reviews here and see that they are having their own people writing up reviews as this is the only explanation for anything greater than a single star review. If you are looking to punish someone, you found the right place.

My brother was transferred to this facility from Presbyterian hospital with Covid. He was fine when he got there. I had asked (including the director) four different people to please plug his phone in. Not one person complied!! On his 3rd night at 2 in the morning I received a call from a nurse saying she had tried both my brothers and my phone numbers and they were disconnected. They were NM phone numbers. The problem is I have one phone number and it is a CA phone number. I asked the nurse where she got these numbers and I was met with condescending BS. So my brother took a real dive and was transported back to the hospital. A few days later I picked up my brothers personal belongings. When I got home, there were articles of clothing and a set of keys that did not belong to him. Sadly, my brother did not recover and passed away soon after. So do me a favor and don't tell me you want to investigate, I am considering contacting legal counsel!!!!

Avoid this facility like the plague. After a hospitalization for COVID we were told the COVID Quarantine facility for Albuquerque Seniors awaiting Assisted Living placement was Canyon Transitional. We were forced to provide payment for the month up front, nearly $10,000. We were not presented with any paperwork and never signed intake papers on our mom's behalf. Our mother experienced neglect due to severe under staffing. While there she was not showered or dressed for 5 days. She experienced a skin tear on her lower leg that is still being treated. She was not allowed a personal visit with her hospice nurse. She was not provided her pain medication as prescribed. When called on this the nurse was rude and stated she needed to ask, I stated that was not what the orders said and begged her to take the meds in and she huffingly agreed.The COVID unit had just one nurse for two hallways of rooms! On the day we were moving her to assisted leaving, although the social worker and nurse confirmed the date and time, our mother had not been removed from her bed when it arrived, she was not dressed, or packed. Our mother was brought out of the facility in the same pajamas she had worn for 5 days, although she had clean, warm clothes in her room. She was brought outside with no coat, no shoes, no blanket on her at 9:45am at 40 degree temps and was left in her wheel chair in pj's and socks next to the transport. The nurse was more concerned with signatures for meds than caring for our mom. I finally took a sweatshirt out of the bag of her belonging and the transport driver helped my mom put it on while I put shoes on. The nurse stood there with her paperwork. This place needs investigation. DO NOT PUT YOUR LOVED ONES IN THIS FACILITY. 4/20/2021 - Additional review - We tried to respond to coroporate office as requested several times with no answer. We were forced to pay for 30 days in advance before they would admit our mother. She was in residence 7 days. Two weeks after she left we called for the refund. We were told it would be processed immediately. Our mother was released 2/13/2021. We still do not have the refund. After MULTIPLE phone calls we were told it was mailed twice to our mother's old home, which has a mail forwarding order, but the refund did not show up. We were then told it was mailed a third time to the correct address, but it still has not shown up. The have owed us a refund of $7,500 for more than two months now. This all sounds like a scam. BEWARE!

My dad was transfered here from Lovelace. The staff here gave more attention to my dad then the other facility. From the front desk to the providers everyone was so caring and helpful. All of our questions were always answered. We felt confident we were leaving my dad in good hands.

Canyon is a beautiful facility with a friendly staff. Great customer service. I was greeted with a huge smile from receptionist who was willing to show me where I needed to go. Admissions director was very professional and friendly. Answered all my questions patiently and with a smile. When I entered the therapy room staff was friendly and smiling as well, Great energy.

We are very frustrated with staff, not answering calls,v been trying to talk to social worker from the day my mother was sent there June 11, 2020,: Social worker has not returned any of our calls, they don't have bedrails around the beds, on June 17 and June 22nd, we got a call in the middle of the night telling us that our mother has fallen and they want to know what they should do? Where are they finding these nurses?; Can't understand them, staff are uneducated and unprofessional, they continue to kept lying to us that she is okay, I jus want to get my mother out of there.

Unpretentious little place. I'm very happy with the room my family member is in. Nice bathroom with shower. Nurses and CNAs have been very kind to my family member and to me. I like that I've been made welcome to visit any time of day or night. I will add more after my family member us discharged from rehab.

Well, they damn near killed me! I'd had a ruptured aneurysm in my head & was sent here for Rehab. I was vomiting & lost 20+ lbs over the 7 days I was there. They lied to my gf & said I was eating well & participating in PT. My gf got me out of there & back to UNMH where they decided I had a complication & needed a shunt to remove excessive cerebrospinal fluid from my brain. I decided not to sue cuz I'm not up to the stress & might not even life long enough to see the outcome!! Nurse Valerie

My aunt stayed in this rehab 5 years ago. Ginger who at that time was the Administrator was a horrible lady. She didn’t seem to care much about patients or staff. I went to her nursing staff with issues and they were never addressed. I went to her with issues and she basically said to me ( if we are not happy that’s not my problem). So we took her out. I guess if you have a head in the seat it doesn’t matter who’s running your building. What a shame.

WARNING!!! My Uncle is currently in this facility and I would NOT recommend it to anyone! My Uncle has fallen twice because of staffing issues. They don't check on their residents. While visiting, I watched a little old man fall right in front of the nurses’ station, from lack of being watched. He was right in front of them!!! My uncle is diabetic and they do not offer sugar free or diabetic friendly diets. They do not check blood sugars and administer insulin or medication on a timely bases. There is no need for the use of call buttons, as they do not respond to them. My uncle has to wait, usually 30 minutes or more, to get help. The nurses’ refuse to do the work of the CNA’s and rather helping, make you wait till a CNA is available. They are not safe and do not use gait belts when transferring. They are more concerned by their appearance than they are the care they provide.

My father spent almost 5 weeks here after a sickness. Nearly everyone was friendly and all were helpful. The physical therapy team was particularly good. A nurse named Andrew was a stand-out employee. The head of the dietary department was so caring and concerned about my dad's health. It was a tough experience for my dad but I would say Canyon did a good job.

Sister is there for rehab . She had call light on for two hrs no one checked on her she had call light on because tv went out. Thank God it wasn't emergency if she had fallen and was on the floor with an injury they took two hours to respond to call light .they play the blame game someone else was to chk on her.. lack of care if you care for loved one don't let them stay here.rehab was great for her . People placed here for care other then rehab. Are not cared for the way they deserve to be cared for. SAD

A family friend is there on hospice suffering from dementia and terminal cancer they lost him and called the cops and then had him arrested! And never notified us what an horrific experience of you love your family you will not put them here!

Finger pointing, excuses, no accountability, poor care, and flat out lies. My MIL came back from a week in their care severely dehydrated and weak from hunger. She had fallen several times and is covered in bruises. We were called at 11pm to inform us she had fallen earlier in the day. Why'd they wait so long to contact us? Nope, never taking her back there for care (or lack there of).

Admittance paperwork not done for nearly 2 weeks. Called many times when no one answered the phone (main line, nurses station and in room). As POA I was not informed in advance of case planning meeting. When my mother (with dementia) left the facility she was wearing 5(!) nicotine patches. I would not choose this facility again.

This use to be a well run nursing home. We have been concerned with the level of care for skilled nursing patients. So many problems: terrible smell, theft of clothing, dirty patients...to name a few. The facility is more concerned with rehab patients than those who are skilled nursing.

Warning If you care for your loved ones, do not take them here. It is over run by inadequate, uneducated individuals. Every time I would call, I felt like I was speaking to a kindergartener. It seems to me that this place needs a lot of staff training in procedure and also hospitality. They did not provide me with adequate updates of my father's wellbeing and their lack of detail and professionalism is truly astounding. My father entered the facility to regain his strength after he had a fall and went into rhabdomyelsis, but also has had some mental confusion. Unbeknownst to me, they sent him via ambulance at midnight back to the hospital. Even though I am listed as his power of attorney, I was not the first contact. I was told that they attempted to call a Susan (who is my mother his ex wife, who also by the way, never received such a call). It just amazes me that no one has returned my call nor have they told me about where his belongings are located. I was relieved to know he was back at the hospital with caring individuals who provide me with daily updates. When I went to pick up my father's belongings, I was looked at like I was speaking another language. One lady was so rude she didn't even look up at us from her paperwork. A gentleman in a wheelchair asked her where to get out, and she just ignored him. If that was my father, I would at least expect her to treat him with kindness. I called to speak to an administrator named Ginger about my experience, but was never called back. I would never recommend this facility to anyone.

My client was there, the staff gave her the wrong meds, also when she called for a drink of water one of the staff members tossed her call button and walked away and did not help her at all. They also stole her belongings.
