I have never seen nor been to a place that is truly horrible. I'm beyond surprised that you are still in business. Your treatment for patients is beyond inhumane. I would highly recommend anyone who decides to use the services of Hamilton Rehab please seek another place for your loved ones.

After only 5 days upon arrival we were informed by the roomate's neighbor that they mixed up the bloodwork of the room's patients which discovered an infection that was only tended to after 2 days of the initial discovery. Never enough staff on premises (2/3 nurses, 4/5 CNA's per 40 patients on the floor we were on) which to their detriment: some were truly genuine and caring; while others you had to monitor or 'suggest' attentive service / care. Agency hired staff meant you never really have consistent staff 45% of the time which leaves unfamiliarity with the patients as they may be there for just that day or brief period of time, so there's no investment of the patient's care to a certain point which was just basic at most (we had a guy work there just 1 day of the whole week). Our patient's condition did get better to a certain extent, but then was replaced by numerous bed sores by neglience of staff; left in a wheelchair from morning to early evening with no changing (leaving the patient sitting in a soiled chair & clothes, never showered until after a month and only because it became questioned and mentioned to 'higher ups'. One time patient was sitting in the dayroom by himself at a table chewing on a cap from an Ensure container and the best response given from the nursing supervisor was 'I didn't think he do that'. At times, bedsores not covered / bandaged while patient sat in their urine and feces in chair or in bed. There were times 'supplies' ran out so CNA would clean patient with body lotion or feed patients on feeding tubes by refilling the feeding dispenser with cans of supplement meals either 'scrounged' from other floors. When complaints were filed or reported to offices or health dept the scramble to make rooms fully stocked, usually having the room cleaned out after a day or two once inspection was done. Staff hours were cut periodically as the nurses became vocal of this st times, as some just resigned. A few were fired for allowing falls (in approx 1 month there were 4 to 5 falls where one patient just never returned... RIP) Some rooms were never fully renovated as you can see the open holes in walls still waiting to be patched up; which is why the facility tours are restricted to certain floors that showcase the 'selling points'. This don't neglect the patients who may get superb care (but that depends on your coverage and finances and how vocal you are cause that can also either have actions taken or retaliation as well) but if you can just avoid this facility (and sadly it sounds like these types of places are all the same) please do your loved one a favor and keep searching, ask other patient's family members if you can. You don't want the final moments or pics (cause this wouldn't have been published without them on file despite the 'notice' on them not being allowed which you can guess why; just read this over again) to be like the ones we live with for the rest of our lives. 2 Stars go to those very few who tried their best. For whatever reason (and sometimes it can be just availability of facilities) you find you loved one here BUT your coverage or insurance allows you to; get 'em out the moment you can.

My father went to Hamilton Park Rehabilitation Center for almost 3 months now. He is a Chinese patient who is very happy and satisfied with the services there because there is Chinese social worker Ms. Guan and Ms. Samantha who helped my father a lot during his stay at the rehabilitation center. Ms. Sisi Yam is the corporate director of Asian initiatives which was very helpful throughout the entire process of transferring from the ICU to the rehabilitation center. Ms. Sisi Yam is very professional and she spend a lot of time explaining everything we need to know about the rehabilitation center before my father was transferred there. So my family had a ease of mind when choosing this rehabilitation center. Ms. Sisi Yam explained how there is a Asian floor with gourmet Asian foods that my father can eat which made him happy. The Asian floor made him more comfortable to be around people who speak his language. Ms. Guan the Chinese social worker has been very nice and friendly to check on my father very often and talk to him to make him feel more comfortable at the rehabilitation center. Ms. Samantha provided exceptional service for my father and always so caring. Overall my father and my family is very happy and satisfied with the excellent Chinese service at this rehabilitation center. I believed we made a great choice in sending my father to this rehabilitation center because he is receiving the best care ever. Doctors, nurses, CNA, and staffs all treated my father very nice and friendly everyday. Speech therapist, Loni helped my father learned how to speak and eat again after his intensive stayed from the ICU. My father felt so happy to eat and talk again after 2 months intubated at the ICU. Loni speech therapist is one of my father's favorite person to see because he can eat and talk during his therapy sessions :) My father was blessed to have a Chinese occupational therapist, Mr. Richard Chan to help him get his muscles stronger daily. And my father is so happy to have a wonderful and caring physical therapist which helped him with daily exercise to strengthen his weak muscles. My father was very weak and need a lot of physical therapy to help him walk and use his hands again. My father loves the 2nd floor gym which has a lot of equipments and machines to help strengthen his muscles daily. Thanks to the wonderful team of therapists, my father is able to function close to normal now. We're also very thankful of the pulmonologist doctor who took really good care of my father and provided him with exceptional care with his trach collar tube. My father finally got his trach collar tube removed last week and he can finally breathe on his own without oxygen after 5 months. I like how the rehabilitation center doctors and therapists plan my father's care because they don't rush his progress. They do everything slowly to make sure everything is done the right way. This is very important because my father feel more safer this way when the doctors and therapists plan his care appropriately to make sure nothing goes wrong. We're able to video chat with my father daily at the rehabilitation center because there is a tablet in each room patients can use. I would highly recommend Hamilton Park Rehabilitation Center to everyone who is looking for a short term or long term for their family member. We had a great experience there and we're so thankful that my father is able to receive all the great care at Hamilton Park Rehabilitation Center now. Our family felt very blessed and happy that my father will be going home soon and continue his happy life with us again. My father said all the doctors, nurses, CNA, & staffs are so nice and friendly to him which made his stay happier at the rehabilitation center. I highly recommend Asian families to chose Hamilton Park because the Chinese service is beyond awesome :) Overall, Hamiliton Park Rehabilitation Center is a great place for anyone who needs care because they provides excellent medical care for patients :) Great services !!!

Our family agree with all the negative reviews but not to 100%. In the Rehab my brother do have some good staffs helping him as he needed. Miss SiSi Yam Corporate Director of Asian initiatives and Social worker Ms.Guan has been very helpful. Miss Yam's knowledge in the hearth care system is above and beyond. We received great recommendation from her to help my brother.

I would agree with all the negative reviews!! I would also give a zero if I could but not at an option. Let me start with saying this place has no straight forward protocols and make up their rules as they go. Let me start by saying they have been shut down for months and not allowing visitation with loved ones when other nursing homes are allowing some visitation. They keep stating that the reason they do not permit visitors is because they keep having new COVID cases which is interesting if they are not permitting visitors? Director is a heartless and rude individual when I had a run in with her. I had gone to see my sick parent who is disabled and now has been diagnosed with stage 3 cancer and on top of all of it from outside the facility! My children and I would just wave to him when he was being taken to his radiation sessions. The director had approached me very rudely and shooed me and my children away. My four year old began to cry. On top of it they say no one is allowed to see him but they were allowing my mom to accompany him to and from his radiation sessions. The nursing staff would call her everyday saying they had no one to take my parent to his treatments so there is a double standard. This is the least of the concerns. Amidst all of this I got a phone call saying they didn't have his chemo pills and he was not taking them for a week when he was supposed to get them with his radiation treatments. They also made a mistake of giving him chemo the first week with no radiation because they couldn't get their facts straight. He also was sent to the hospital too late with low oxygen and now is fighting for his life due to negligence on the weekend nursing staff!! We found out he was choking on food for almost two days but being treated for pneumonia. Due to all of this his cancer treatment was interrupted and as a result we were given a terminal diagnosis and we had to place my dad in hospice. I don't think they really cared what happened to my dad. Weekend staff is horrible!!! The nurses are very sweet when you talk to them. Some of them go out of their way but others you feel don't want to be bothered with you or what issue your parent might be having. The only reason my parent is there is because we live so close and want him to be near to us. They have you quarantined away from seeing them and they don't really answer the phone or give you much in the way of updates. They don't want to be bothered but then when you find out your parent could have been in real danger that's when this is unacceptable to me. The director, Miriam, talks to you condescendingly and if she does not like the patient they are doomed!! Please if you have another choice do not choose this facility! This is not the end I will be taking legal action and even contacting department of health in response to all of this. Hope no one ever treats these facilities elders in such a manner. Please do everything you can before sending your loved one here!

4 stars for the physical therapy department as they genuinely care for my mother and are always willing to go the extra mile. but if you need anything your better off going to the receptionist then to the nurses on the floor. she’s very kind and helpful but can’t say the same for the night guy. nurses do try but are understaffed. food is below average.

For me I appreciate the experience and the equipment to enhance success of quality of life. The environment is friendly and productive.

This website is from the day they of construction. My grandmother is currently there and each floor is worse than the next. Dirty and old rooms and not a place I would recommend anyone’s loved ones to go. Unless there was no choice. Looking to move my grandma ASAP

The place is decent enough in terms of they do what they need to do and my dad has been taken care of. They offer activities for the patients. My problem is the apparent shortage on nursing or whatever the issue is there. Every time I go to get the new phone number when my dad is moved to another room or go the the nurse for information on him, I'm usually redirected or given vague responses and I am allowed to have his info. It took me 5 phone calls tonight to get his correct phone number. A PATIENT wound up trying to help me because that's who the operator transferred me to. Ridiculous. Also happens when I try to call and speak to his nurse. And they brush you off the phone quickly. I understand they are busy but I don't call constantly, as I generally talk to him direct. Also, I was called last week, I was supposed to be called about how my dad is doing and I wound up being harassed (like 3 people on the line) and they tried to bully me into how much money he has in his accounts. They are ready to kick him out sooner than his insurance should cover. They claim physical therapy cleared him , yet they want to know the "plan" we have for him, like they really care about his well being. When I was recently there, he was still a FALL RISK so I don't understand how he is cleared. His insurance should cover another week otherwise, but we(my sister and I ) were threatened numerous times he will be cash $500/day until we are able to get him next week. Leaves a bad taste in my mouth. I'm really furious right now with this place. They need to learn that it's very stressful on the families as well and just because insurance stops paying , the person isn't automatically ready to be on their own.

Three stars ARE not for the nursing staff, rehabilitation staff the best. That would be the only reason to go there. Was told how good they were before. To bad some nursing staff makes all look bad. Patients, I DID not known have to ask for their meds. A bell at beside, I thought, meant someone could help u, they are there for that!!! Understandable if busy, but to come in maybe30-60 min later asked what is wanted, then NOT COME BACK. COME on this is their job, they have the job because as patients we need them. Also they are not helpful for the patient in navigating all that needs to be when newly arriving. The kitchen staff also needs help. Underlying circumstances sometimes require looking into. NOT everyone goes in with the same dietary needs. I AM not there because I want to be, and I AM greatful for having a place to help me recoup. Work with your patients.

The place is very nice however the majority of the nurses aids need to be replaced. Patricia from admissions is an absolute doll as well as the girl at the front desk. They could use more dedicated staff but aside from the aids most people are great.

My mom needed sub-acute care in late September after a mild stroke, and we brought her to Hamilton Park after recommendations from friends and visiting the facility. Everything was very, very good. The PT/OT staff were excellent, getting both types of therapy five days a week, and one on Saturday and the other on Sunday. They even had therapy on holidays! The nursing staff was good, and the nurses aides were excellent, caring people. I even thought the food was good (the breakfasts are feasts!). We took my mom home on January 1, and the discharge process (meeting, post rehab care and arrangements) was organized and well done. Based on some of the stories I've heard and facilities I've seen, I highly recommend Hamilton Park

My father in-law was there and when he got there, he used a Walker. When heleft, legs were in bent position, no longer able to walk and bed sores. Not all the nurse were negligent, but he received inadequate care.

The food in here is the lowest grade allowable by law I am left in wet and soiled diapers for over an hour or left in them to eat meals because they are ridiculously under staffed . My medication is continously late .The two stars are for the PT and OT staff who were excellent.

This is A TRULY AMAZING PLACE. You have no idea how surprised my wife & I are to have chosen this facility for her rehab. When we were given the list of places to choose from, this facility had a "Below Average" rating by CMS. This concerned me a bit, but we decided to go here anyway. What a great decision this was! We found out that it used to be poorly run when it was previously owned by Victory Memorial, but now that SUNY Downstate took over, the difference is incredible. You will see some really bad reviews, which I've also read, but don't be discouraged. These were probably written prior to the new management taking over, or by someone who couldn't get a bed here or who's insurance doesn't cover this grand facility. That's a real shame, because I doubt that anyone can find much, if anything, to complain about Hamilton Park. Feel free to check it out for yourself if my review doesn't convince you. Visiting hours are 24/7, the facility itself is really nice, almost like a hotel, with some good views of the Poly Prep Campus & its ducks, & the staff is phenomenal. We also like it because it's close to our house, but that's just icing on the cake. I visit my wife every single day, at various times, & we've never had any issues. Free street parking isn't hard to find, & it gets easier the later you go. Starting with the security in the lobby, my kids & I always feel safe & secure, no matter what time we visit. There are also security cameras all over the place. The entire facility is very clean & well-kept, & the food, my wife tells me, is delicious. She's on the third floor, & there are four lounges on that floor alone. They have television sets, comfortable chairs & sofas, reading material, snacks & drinks, & board games. They also have several activities during the day, such as Bingo, as well as shower facilities near each group of rooms. Whether it's 2:00 in the afternoon, or 2:00 in the morning, my wife is very well taken care of. The workers are very friendly, like they enjoy being there, with no attitudes or acting as if you're bothering them when you need to call them regularly. They do get busy at times, but try their best to get to each & every patient in a timely fashion. Just by visiting, I've seen first-hand how hard the staff works, & just how well-trained & dedicated they are. Headed by Jovelyn Calderon, the nurses form a terrific team, which includes Ash, Spence, & the one and only Betty Jacques. The doctors, such as Dr. Funaro, make their rounds to see the patients, & the aides, like Marie, also lend a hand in the patients' comfort.Then there's the physical therapy that my wife has 6 times per week in a spacious area which covers most of the 2nd floor. There, she is instructed by a physical trainer who is second to none, in Chris (Christian). This man has worked wonders with my wife, enabling her to go from a walker to a cane to using nothing at all to move around. He is excellent at what he does, as well as kind & considerate. He not only builds up her physical abilities but also her confidence. He is very professional & always there to assist her. We can't thank him enough! The scheduling is fairly flexible, & the equipment is very modern, like the $25,000 anti-gravity treadmill that my wife gets to utilize.She also engages in occupational therapy 6x a week,with another group of outstanding personnel in Nadia, Shaq, & Denise.Some of the staff live locally, but all have been nothing but wonderful to my family & I...the social workers,maintenance workers,& everyone else(I don't want to miss anyone because we haven't come across anyone who we haven't liked).This facility seems to be like a hidden gem,nestled between the SUNY Downstate Medical Center & a huge employee parking garage.My wife is being released in a few days,after deciding to stay for an extra week,& we'll have nothing but really good memories of a necessary process that was made as pleasant & care-free as possible.I would easily give this center & its people 10 stars if I could.THANK YOU TO EVERYONE at Hamilton Park & KEEP UP THE GREAT WORK!

The nursing is well under - staffed, the security on this facility is ridiculous (and too busy watching movies on their cell phone) as well as anyone can walk in and out of this build without question.
